“MANAGEMENT OF A PRETERM LOW BIRTH WEIGHT BABY”

“PRETERM LOW BIRTH WEIGHT BABY”

ABSTRACT:

Baby of Sima, weight 1.5 kg, sex -male was born with respiratory distress syndrome delivered by caesarean section with the indication of 2nd gravida with 35 weeks pregnancy with Intra uterine growth retardation with foetal distress.

We meassured the APGAR score :
We found that —
the appearance of the baby was cyanotic,
pulse rate was 80b/min,
baby cried when stimulated, body movement was not so good, respiration was irregular.

We took some steps here for first line management:——
. O2 inhalation 1L/ min.
. wrapped the baby with cotton for warm.
. Inserted a cannula for fluid and drug management.

Then we sent the baby in a tertiary level hospital for NICU support as it is an Upozilla level hospital and there was no NICU support.
At that time the baby’s clinical condition was:

SPO2 : 95%
FIO2: 40%
And the vital signs were stable.

In NICU the baby was given the following treatment :

. C -PAP care.
. NG tube feeding 12 ml every 3 hourly.
. Inf. 10% Baby saline.
. Drop Loracef 35 mg (0.35 ml) 12 hourly by
NG tube.
. Syp. Frusin (0.2ml) once daily.

It was diagnosed as a case of
PRETERM (35 wks), SGA( 1620gm)
with RDS with moderate PDA with mild PPHN with PFO.The mother of the baby Sima had the history of repeated pregnancy loss.She also gave birth on IUGR baby through per vaginal delivery but the baby was dead 2 years back.

When she came to us we recorded her all vitals :

. BP -100/60 mmhg
. Pulse -62 b/min.
. Spo2 -97%
. Anemia – present.

By Ultrasound profile : —–

We found a single alive pregnancy about 35 weeks with foetal cardiac pulsation and foetal movement present, AFI – Adequate and Breech presentation with IUGR.

After 2 days of NICU support the baby came to us and we supported the baby with proper mamagement :
. O2 inhalation.
. Wrapped the baby with cotton.
. Breast feeding and medication through NG tube.

At 7th post operative day, the condition of the baby had improved.The bowel and bladder habit was normal, the body movement was good and the baby responsed our command, sucking reflex was also good.
We released the baby at 7th post operative day.
